Six steps, four weeks minimum at each, 2.5 mg through 15 mg. The intervals are minimums before increasing rather than instructions to increase — holding longer is routine and is how most prescribers manage tolerability.

The units table only applies if you are drawing from a vial. Pens deliver a fixed dose and require no calculation. Note how far apart the columns are: 5 mg is 100 units at 5 mg/mL but only 25 units at 20 mg/mL. Reading the wrong column is a four-fold dosing error.

Check the concentration printed on the vial you are actually holding. The same dose in milligrams is a completely different mark on the syringe at a different concentration. If the number here does not match what your prescriber told you, trust your prescriber.
Common questions
- Is the chart different for Mounjaro?
- No. Same molecule, same schedule, same steps. Mounjaro is labelled for type 2 diabetes and Zepbound for weight management, but the escalation is identical.
- What are the maintenance doses?
- The labelling identifies 5, 10 and 15 mg as maintenance options. Which one a person stays on is a clinical decision, and staying below 15 mg indefinitely is common.
- How long does the full escalation take?
- Following the minimum intervals, reaching 15 mg takes at least 20 weeks from starting. In practice it usually takes longer because steps get held.

- The prescribing information lists 2.5 mg once weekly as the starting dose, held for the first four weeks before any increase.
- What is the maximum labelled dose of Zepbound?
- 15 mg once weekly, reached through the stepped schedule shown above.
- How many units is 5 mg at 20 mg/mL?
- 25.0 units on a U-100 syringe. The arithmetic is (dose ÷ concentration) × 100, and it only holds if the concentration on your vial matches.
