GLP-1 fatigue

Fatigue is in the labelling. It appears in the Wegovy adverse-reaction table at 9–11% depending on the dose against 5% on placebo, and among the reactions Zepbound lists as reported in at least 5% of patients. So it is documented, and it is reported above the placebo rate.

These products carry a boxed warning regarding thyroid C-cell tumours observed in rodent studies, and are contraindicated in people with a personal or family history of medullary thyroid carcinoma or MEN 2. What the labels do not do is explain it, and there are at least three candidates they cannot separate: the drug itself, eating substantially less than before, and losing weight quickly. All three arrive at the same time on this treatment, which is precisely why the trials could not disentangle them either.

The reason that distinction is worth caring about: two of those three are things a clinician can check. Intake that has fallen far enough to matter, and the nutritional consequences of rapid loss, are examinable. Fatigue attributed to the drug and left there is fatigue that never gets those checks.

Common questions

When should I raise it?
If it is limiting what you can do, or if it arrived alongside anything else. Fatigue plus dizziness, or fatigue with an inability to keep fluids down, is a different presentation from feeling flat.
Does it settle?
The labels record how often it was reported, not how long it lasted. They contain nothing on duration in either direction.
Could it be something checkable?
Thyroid function and iron status affect energy, are testable, and do not stop being possible because you started a GLP-1. Same reasoning as the hair loss page: timing is not attribution.
