Oral GLP-1s changed this question recently. There is now a tablet labelled for weight management, not only for type 2 diabetes, which means "the pill" is no longer automatically a diabetes prescription being used off-label.

Without coverage, the decisive question is which indication you are being prescribed under, because that determines which product and which self-pay programme you are eligible for. An oral product approved for diabetes is not a cheaper route to a weight-management prescription — it is a different labelled use.

The routes people actually pay through, and what determines the number in each.
Through insurance (copay)
If your plan covers the drug for your indication, your cost is the plan copay. Coverage for weight-loss indications is far less common than for type 2 diabetes.
Telehealth platform (brand drug)
Online clinics bundle a prescriber visit with brand-name medication. Membership fee is usually separate from drug cost.
Compounded semaglutide / tirzepatide
Compounded versions are prepared by pharmacies rather than the original manufacturer. Availability and legality depend on FDA shortage status, which has changed several times — verify current status before relying on this route.

One thing that surprises people comparing oral and injectable prices: the milligram figures are not comparable across routes. Oral semaglutide is absorbed far less efficiently, so its maintenance dose is 25 mg daily against 2.4 mg weekly for the injection. Cost per milligram is therefore meaningless between them — compare cost per month instead.

Common questions
- Are pills cheaper than injections without insurance?
- Not something this site publishes figures for, because pricing changes too fast to quote. Run both quotes through the calculator on a cost-per-month basis, which is the only comparison that holds across routes.
- Is there a tirzepatide pill?
- No oral tirzepatide product is approved. Tirzepatide is injection-only in both its brands.
- Do the tablets have extra requirements?
- Yes, and they are not optional. The label directs taking them on an empty stomach in the morning with no more than four ounces of water, swallowing whole, then waiting at least 30 minutes before food, other drinks or other oral medications.
Common questions
- Why do quoted GLP-1 prices vary so much?
- Because the quotes are not measuring the same thing. A membership fee, a per-vial price, and a per-pen price all describe different quantities, and the dose you are on determines how long that quantity lasts. Converting everything to cost per milligram is the only way to compare them directly.
- Does insurance usually cover this?
- Coverage is far more common for type 2 diabetes indications than for weight management. Whether your specific plan covers your specific indication is something only your plan documents can answer.
