Why do quoted GLP-1 prices vary so much?

Because the quotes are not measuring the same thing. A membership fee, a per-vial price, and a per-pen price all describe different quantities, and the dose you are on determines how long that quantity lasts. Converting everything to cost per milligram is the only way to compare them directly.

Does insurance usually cover this?

Coverage is far more common for type 2 diabetes indications than for weight management. Whether your specific plan covers your specific indication is something only your plan documents can answer.

There are now two FDA-approved oral semaglutide products, and they are not interchangeable: Rybelsus, indicated for type 2 diabetes, and Wegovy in tablet form, indicated for weight management and added as a dosage form in 2026. Most pages still describe the oral option as a single diabetes product, which is out of date. Anything else sold as a GLP-1 pill is either a compounded oral preparation or a supplement using the term as marketing — different regulatory categories, not different prices for one thing.

Every approved route for a GLP-1 receptor agonist

The complete list. Anything not on it is not an approved GLP-1 medication.

Subcutaneous injectionSemaglutide, tirzepatide, liraglutide, dulaglutide and exenatide are all administered this way, weekly or daily depending on the product.
Oral tabletSemaglutide only, as Rybelsus. It is the sole approved GLP-1 receptor agonist that is not injected.

There is no approved transdermal patch, sublingual drop, capsule sold over the counter, or topical preparation delivering any GLP-1 receptor agonist. Two routes is the whole list.

Here is the part that is genuinely dangerous to get wrong, and it is specific to this drug rather than a general caution. Wegovy exists as both an injection and a tablet, under the same brand name, containing the same molecule — and the two run on milligram scales that differ by roughly a factor of ten. The injection tops out at 2.4 mg weekly. The tablet's maintenance dose is 25 mg daily. A number carried across from one form to the other is not a small error.

The reason for the gap is absorption. Swallowed peptides are largely destroyed before they reach circulation, so an oral dose has to be far larger to deliver a comparable amount. That is also why the two approved oral products have their own separate schedules rather than sharing one — below are both, on their own scales.

Oral semaglutide also carries an administration requirement the injections do not: taken on an empty stomach with a small sip of water, waiting before anything else. That instruction exists because absorption depends on it. It is a daily constraint for the life of the prescription and belongs in any comparison against a once-weekly injection.
